Insurance

The Men’s Health Place provides high-quality care to folks with and without insurance. Please see the list below of in-network insurance providers. Although we do not accept all insurance plans at this time. We are willing to work with you on submitting claims for out-of-network insurance plans for partial reimbursement, by supplying a superbill for you. We do not currently accept Medicaid or Medicare plans, nor can they be reimbursed by a superbill. Please check in with your insurance plan for additional information by calling the number on the back of your insurance card.

Fee for Service

• Initial Medical Evaluation: Price varies by insurance for 30 minute initial appointment.
• Routine Follow ups: Price varies per insurance for 20 minute follow ups. If additional time is needed, you can be scheduled for an additional visit.
• Cash Based Appointments: A reduced rate is applied when paid prior to your appointment: $150 initial, $125 per follow up.

Good Faith Estimate and Frequency of Visits

This is only an estimate, subject to your status, preference, and provider preference:
After your initial diagnostic appointment/consult, a follow up appointment is typically recommended one month later, then every three months.
Subsequent visits can be fourteen to thirty days after any medication change or period of instability.
If stable on mediation(s), follow up appointments are typically recommended every 3-4 months for the continuation of care and medication refills.
You may see us more often per your preference in any scenario.
If you have questions about the frequency of those visits, you can reach out via email, by phone, or portal message. We will request a visit if it has been longer than the above frequency.
